Adult Learner Intervention Interview and Screening Process

1. Print or complete the online intake form located below. The online form is automatically submitted to PANDA staff. Fax the printed intake form to PANDA at the number listed in #2.
2. Have a student complete the consent form below and fax to PANDA.  Fax (763) 504-4096.
3. Once both forms are received, PANDA staff will contact you to arrange a phone interview with your student.
4. PANDA staff will send recommendations to the teacher.
